With a doctor's or dentist's referral, our services are covered, at least in part, by most health insurance plans. As a courtesy, we call your insurance company to verify physical therapy benefits and obtain necessary authorization prior to the start of your treatment. Workers compensation claims require pre-authorization. Our front office will work with you to obtain approval to receive treatment in our clinic.

Berkeley Physical Therapy is a contracted provider for many PPO and HMO health plans, including Medicare, Blue Cross, Blue Shield, United Health Care, Benefit Risk Management, Health Net, Great West, Private Healthcare Systems and Alta Bates Medical Group. We are also a member of the Physical Therapy Provider Network, America's largest network of rehabilitation professionals in private practice.

Copayments and self pay fees are collected at the time of your physical therapy visit. Payment can be made by cash, check, credit or debit card.
